Our clinical nursing staff and wider health care team can also assist with high intensity supports in the community as well. This will typically involve the development of a person-centred and best practice clinical care plan, which will help our staff to monitor your health and provide support . We will do a :
- Nurse Visit and assessment and vital signs monitoring
- Case management
- Rehabilitation post hospital
- Overnight nurse stay and care
- Advice and management of chronic illnesses
- Palliative care support
- Ventilator Management and support
- Colostomy and Stoma care and support
- Diabetes Management and educational support
- Wound management including care plan regimes
- Set up medication regimes and administration with supervision
